New Flu Vaccine Consent Forms Required

Almost 500 of you signed consent forms giving permission for your child/children to receive Influenza (flu) vaccine here at school through the New Mexico Department of Health’s School Kids Influenza Immunization Program (SKIPP).  In the past 1 -2 weeks, we’ve been informed that the consent form for this year has been changed and none of the consents you’ve signed are valid.  The new form must be used for your child/children to receive flu vaccine here at school at no cost.

Your student will receive the new form for you to complete. Please return the form to the school with your student next week. You can also download and print the consent form here. Click to Download the Consent Form 

Since so many of our students live in the dormitories, SFIS provides the ideal environment for the spread of contagious illness: a large number of people sharing a relatively small amount of living space.  The flu vaccine is a proven method to prevent the likelihood of your child/children becoming infected with the influenza virus.
