2nd Semester Return to School Mandatory Student and Staff COVID-19 Screening Tests

In order for you student to test and return to class, please send your student(s) with the following documents in-hand tomorrow, Monday 1/31/22.

1. A QR code accessible on their phone that was created during the PMG registration process.
   Tip: Save their QR code registration as a photo on their phone for future use- they will need it.
2. A signed parent consent form

Q: I don't have a QR code for my student because I haven't registered your student yet.  What do I do?
A:  Follow these instructions:

• Scan the following QR code or go to this link:http://pmg.app.perseus.id/div/sfis
• Complete the online intake form
• Set up Health IDs for each student
• Use the phone number you want to receive the results on
• Students born prior to 2008, the parent will have to register themselves first and add the child/student as a companion.   All students born after 2008 can register on their own.
• Be sure to sign up for the “Screening Testing Program”
• NOTE: Each student registered to get tested will get an individualized QR code. That student can reuse the same QR code each time they get tested.
