Booster Shot Clinic

SFIS is hosting a Booster Shot clinic for students and staff on Friday, March 18 at the School Health Center. PARENTS OF MINORS UNDER THE AGE OF 16 MUST SIGN THEIR STUDENT UP FOR THEM TO PARTICIPATE.

Parents of students under the age of 16 must provide consent for their student to receive the booster shot by following the steps above. There will be paper forms available for students 16years and older and staff/faculty as they are old enough to consent for themselves to receive a COVID 19 vaccination. 16+ yrs older participants can also follow the steps above to complete an electronic consent. SFIS is requiring all eligible students and staff have a booster shot (3rd COVID 19 vaccination). They will have both Pfizer and Moderna immunizations.
