Annual Physical Exam and Clearance for Physical Activity. You may use this 1-page form, or the NMAA Physical Exam Form. 

NMAA Physical Packet
Annual Physical Exam packet for students who plan to participate in SFIS Athletics. SFIS Athletics prefers the use of this packet, but will also accept the Annual Physical Exam Form.

Annual Registration Instructions

NASIS Portal Activation and Registration                                                                Parents/Guardians of new students who were accepted for admission to SFIS for the 26-27 SY will need to activate their NASIS Portal account to complete the Annual Student Registration. Admissions should have emailed you your Activation Key. Follow the NASIS Portal Activation and Registration instructions to begin.  

Accessing the Annual Registration in Your NASIS Portal                                      Parents/Guardians of returning students who have an active NASIS Portal account can use this information to find the Annual Registration in their portal account. 

Contact Admissions if you:

1.) Need your login information, or                                                                          

2.) Are a new to SFIS and need an Activation Key to start an account, or          

3.) Clicked "Submit" before finishing your registration and need us to un-submit the registration so you may complete information, or              

4.) Require an appointment to complete the registration over the phone or in-person.
