Required Documents

• Birth Certificate

• Tribal Enrollment Verification

  • Verification must be from a federally recognized tribe
  • If the applicant is not the enrolled member, you may use a verification for:
    • Applicant's birth parent, or
    • Applicant's grandparent
      • Lineage must be established through birth certificates

• Teacher Recommendation Form 

  • The form is required and is available in your application account.
  • A letter of recommendation will not be accepted in lieu of the Teacher Recommendation Form.

• 25-26 Grade Report

  • If you are applying for the 7th, 8th, or 9th grade you will need:

    • 25-26 Report Card

      • Report card must include Q1, Q2, and Semester I grades
      • Must be an official report card. 

  • If you are applying for grades: 10, 11, or 12, you will need:

    • 25-26 Transcript

      • Must include Semester I grades
      • Must include credits earned
